What bankruptcy forms do I need to complete?
If you are considering petitioning for your own Bankruptcy then you’ll need to complete the following Bankruptcy forms:
- Debtor's Petition form (6.27) - which is your request to the court to be made Bankrupt
- Statement of Affairs (6.28) - in which you’ll give details about your assets and debts as well as your income and expenditure.
If a creditor has petitioned for your Bankruptcy then you will need to complete a Bankruptcy Preliminary Information Questionnaire, the purpose of which is to give the court more information about your financial affairs.
The Bankruptcy petition forms are no longer available to be completed online, however, you can download and print off the forms from the Gov.uk website.
