What bankruptcy forms do I need to complete?

If you are considering petitioning for your own Bankruptcy then you’ll need to complete an online application form.

If, after answering a number of questions, Bankruptcy appears to be appropriate for your circumstances, you will be provided with a link to the “Guide to bankruptcy”. You will also be prompted to take free independent advice from a list of free advice agencies before proceeding; however there is no requirement to take advice before completing and submitting the form.

After creating an account, you will be provided with an application number by email. This will allow you to log in and save your details, which you can do as many times as you need to.

If you do not have access to an email address, you will still be able to create and submit an online application. However, the process will take a little longer, as your application ID number will need to be created and sent to you by post before you can log in.

There is no paper equivalent to apply for bankruptcy and no ability to apply at a court.

Once you have logged in, you will be asked to provide information about your employment, debts, assets, income, expenditure, bank accounts, legal proceedings as well a brief debt history, all of which an adjudicator will take into account when considering your application. The form is designed to show relevant sections only; if, for example, you do not have income from paid employment then you will not be asked about it. Guidance on how to fill in each section will be provided on screen during the application process.

If a creditor has petitioned for your Bankruptcy then you will need to complete a Bankruptcy Preliminary Information Questionnaire, the purpose of which is to give the court more information about your financial affairs.
